Integrated Service for Disabled Children - Disabled Children (Social Care)

Service Description

Depending on level of need, services provided by the team include:

Types of Provision

Type of Provision Description Contact and Referral Details
Support available to all children and young people, in localities were appropriate. No Salt Way Team service.  
Support for those children and young people with additional needs who need targeted services.

No direct service apart from advice and signposting.

May get occupational therapy service.

Team member may attend LPG.

For those children and young people with multiple needs who need specialist services.

Those not making progress after support as above, or because of the severe nature of their need.

Initially:

  • Decision made within seven working days if referral meets criteria
      - Decision made for further assessment
      - Core Assessment, which is completed in 35 working days
  • Child seen within seven working days as part of Initial Assessment (IA)
  • Preventative services may be provided to reduce impact of child’s disability
  • These services could be identified and provided in conjunction with universal services such as:
      - Access to community resources
      - Multi-agency working with appropriate professionals
  • Once IA completed may refer back to LPG
  • Access to support agencies
  • Referred to Team via Common Assessment/LPG
  • Team member will attend LPG meeting
  • OT services

Plus in some cases:

  • Decision made within 24 hours
  • Child seen within seven working days as part of Initial Assessment
  • Allocation to social worker
  • Core or specialist multi-agency assessment
  • Service guaranteed

For those children and young people in need of immediate care and protection who need specialist services.

Those with the most severe needs after support as above, or because of the acute nature of their need.

Possible Service Response – will vary according to need

  • Core or specialist multi-agency assessment – completed in 35 working days
  • Immediate response – child seen the same day as referral.
  • Allocation to social worker.
  • Service guaranteed
  • OT services
  • Emergency action taken to safeguard and protect children and young people, including accommodating or seeking family/community placement
