Immediate Care and Protection

Integrated Service for Disabled Children - Provision for Immediate Care and Protection

This level of service is available to those children and young people in need of immediate care and protection who need specialist services.

The service is provided via the following services:

Service Description Contact and Referral Details
Clinical Care Very complex health needs, multi-agency care package. Referral to Children’s Continuing .Care Swindon PCT by professional staff working in health, social care and education services locally or through health care teams elsewhere in the country.
Disabled Children (Social Care)

Possible Service Response – will vary according to need

  • Core or specialist multi-agency assessment – completed in 35 working days
  • Immediate response – child seen the same day as referral.
  • Allocation to social worker.
  • Service guaranteed
  • OT services
  • Emergency action taken to safeguard and protect children and young people, including accommodating or seeking family/community placement

Education Support

Individual Assessment.

Observation and advice.

Statutory Advice.

Advice for Annual Reviews and transition.

Regular monitoring.

Staff training with specific individual child focus.

Where child or  young person has the following education access needs :

  • Social Communication and Interaction Difficulties, including Autism
  • Special Educational Needs/Information and Communication Technology/ Alternative and Augmentative Communication
  • Specific Learning Difficulties/Dyslexia
  • Physical Disability
  • Hearing Impairment
  • Visual Impairment
Through school or setting where child or young person is registered.
Koalas

Work in team around the child when children are identified as ‘in need ‘ or have Protection plan.

Pre-school group for disabled children where children can have a maximum of twice a week.

Plus:

Key working for individual children if requested including Team around the Family meetings.

Parent referral or professional request for service to the Salt Way allocation group or directly to Koalas on 01793 465361.
Learning Disability

Occasionally involved with children and young people who are looked after.

Intervention is negotiated with individual families based on the presenting need.

The length of intervention is often long term (until the young person turns 18 years of age) although the frequency of contact will vary dependant on the existing need.

We may offer a service from a single profession - e.g. community nursing, although children and young people who present with more complex behavioural or mental health needs are likely to receive intervention from the Clinical Psychologist and/or Consultant Psychiatrist as well.

Consultant Psychiatrist may be involved in consultation role for in-patients services.

An open referral system is in place and referrals are welcome from anyone. Team Secretary:

Paediatric Therapy

Occupational Therapy and Physiotherapy

Therapists' Involvement

Difficulties and circumstances experienced by the child – related to Physiotherapy and/or Occupational Therapy – need direct therapist intervention to identify and prioritise immediate need.

Intervention will be to resolve immediate needs.

Programme of further management and intervention is developed.

Description of Intervention

Full assessment and evaluation of need.

Development of case management plan.

Training and teaching in appropriate environment.

Collaboration and partnership with child, parents and other agencies to resolve immediate issues.

Assessment and training in use of adaptive equipment

Level of Intervention

Individual intervention or work with parents and carers.

Multi-agency liaison.

Therapy input - minimum of one session per week, direct or indirect.

Children are not expected to remain with this support.

Initial referral from GP or consultant.
Portage Work in team around the child when children are identified as ‘in need ‘ or have Protection plan.

Parent referral or professional request for service to the Salt Way allocation group.

Speech and Language Therapy Work in team around the child when children are identified as ‘in need’ or have a Child Protection Plan, for children who are already known to us.
