CitizenCard - How to Apply

In order to apply for a CitizenCard you will need each of the following:

1. An Application Form

You can apply entirely online or download an application from at the CitizenCard website (www.citizencard.com).

If you are aged under 16 you will need the consent of one of your parents or guardians.

3. A Referee

Every applicant’s details are verified with a referee before a card is issued. You will need to provide the name, address and work phone number of a referee who can verify your details.

4. Two Passport-size Photographs

The referee is also required to sign the back of one of the photographs.

5. Payment

If you apply entirely online through the CitizenCard website, the cost is £12.

If you wish to fast-track your application for delivery in five working days or less, an urgent application surcharge of £26 can be paid.

Sending Your Application

Application forms, photographs and any payments required must be returned to the CitizenCard Card Centre to be processed - please see CitizenCard - Contact Details.

Privacy

CitizenCard is registered under the Data Protection Act and guarantee that they will not pass on any of the cardholder’s details to any other organisation or body. To view the full Data Protection Statement please see the CitizenCard website (www.citizencard.com).

Warning

Every single application form is verified. Providing false information is a criminal offence.

CitizenCard Ltd will always prosecute anyone attempting to obtain a card by deception.
